Welcome to Family First Dental! As a new patient, we ask that you fill out the New Patient Forms below prior to your scheduled visit. This will help speed up your check-in process when you arrive. What to Expect
Don’t forget to bring your dental insurance card! Also, an early arrival is recommended so you will have plenty of time to fill out any necessary paperwork. You can submit your new patient forms before you arrive to save you time during check-in. Please provide a list of medications you are currently taking. Your new patient visit will begin with a comprehensive oral evaluation. A customized treatment plan will be put together with any recommended treatment. We’ll discuss your dental goals and answer all of your questions.
Our team will be able to go over your insurance benefits and help you understand your coverage. Please bring your insurance card when you arrive and we recommend reviewing your benefits prior to scheduling your visit. We will provide an estimated amount covered by your insurance company based on the information provided by your insurance. This estimate is not a guarantee of benefits of the amount that will be paid. We are happy to file insurance claims on your behalf. If your insurance company pays a different amount than originally estimated, you will be billed the difference or provided with a credit on your account if there was overpayment. We can go over our financial policies before any treatment is given.
We ask that you give a 48-hour notice if you need to reschedule your visit. A notice less than 24 hours before your scheduled appointment may result in a cancellation fee. Thank you for your help and understanding.
